The cam turns a splined driveshaft connected to an arbor on the distributor. The arbor contains an optical wheel which sends signals to the pcm telling it the exact location of the cam and crank. This arbor also holds a rotor which distributes the high voltage to the individual cylinder leads on the distributor cap. It's pretty simple really
