I have driven my car the exact same way, but I will be swapping the clutch for the 3rd time in the last ~40K miles, all with a broken TOB.

similar symptoms and my outcome was that the clutch fork actually warped the throw out bearing and eventually broke it. I had metal shavings in the housing where the slave sits.

Damnit... I think I know what's going on now. Let me know if this makes sense to you.

I think the throw out bearing is warped. The outer lip is elongated to where it is rubbing the pressure...

Everything looks ok with the slave. I replaced that about 12 months ago. If I got some kind of warranty on it ill see if I can get another just to see.
